REGISTRATION OF VITAL EVENTS

The registration of vital records is applicable solely to Lebanese citizens.
Palestinians holder of travel documents issued by Lebanon can not register their vital records but can instead legalize them at the Consulate.

REGISTRATION OF MARRIAGE

Required Documents

  • An application form (Click Here to Download).
  • The official American marriage certificate.
  • A recent Extract of Civil Status (اخراج قيد فردي) or a Family Extract (اخراج قيد عائلي).
  • A birth certificate for non-Lebanese spouses (an official translation to Arabic is required if the certificate is neither in English nor in French).
    A return self-addressed stamped envelope.

REGISTRATION OF BIRTH

Required Documents

  • An application form (Click here to Download).
  • The official American birth certificate.
  • The parents’ Family Extract of the Civil Register.
  • The parents’ marriage has to be registered prior to the registration of their children. However, if the marriage has not yet been registered, it is possible to register the marriage and birth at the same time.
  • A return self-addressed stamped envelope.
  • A notarized copy of the birth certificate for non-Lebanese mothers (An official translation to Arabic is required if the certificate is neither in English nor in French).

REGISTRATION OF DIVORCE

Required Documents

  • An application form (Click here to download).
  • The final divorce act issued by the relevant American court (certified copy. from the superior court), with its official translation to Arabic.
  • A recent Family Extract of Civil Status.
  • A power of attorney granting a lawyer or representative in Lebanon the right to follow up on the finalization of the divorce in Lebanon.

REGISTRATION DEATH

Required Documents

  • An application form (Click here to Download).
  • The official American death certificate.
  • The Extract of Civil Status of the deceased, or his/ her Family Extract, or his/her Lebanese Identification Card.
  • The Lebanese passport belonging to the deceased. The passport will be cancelled and mailed back.
  • A return self-addressed stamped envelope.
